Our pledge for Bexley

What we do

The Bexley Wellbeing Partnership (BWP) brings together 17 local health and social care organisations who are committed to working together to improve the health and well-being of people in Bexley.

The BWP work together to understand the local population and agree on how best joint resources can be used to achieve better health outcomes for residents.

Our vision is to improve the health and wellbeing of residents across the course of their entire life – supporting people to start well, live well and age well.

Aims and objectives

  • For services to be developed with the person at the centre, coordinated and tailored to the needs and preferences of the individual, their carers and family at place.
  • Delivering joined-up health and social care services that best meet the needs of Bexley’s population and help to improve the health and wellbeing of residents
  • Improving the emphasis on the prevention of ill health and promotion of health and wellbeing
  • Narrowing the inequalities gap in health outcomes between the most affluent and most deprived and underserved communities across Bexley.

Including people with lived experience and their families, carers, seldom heard, marginalised and underserved communities, is critical to the work of the BWP

How do we do it?

  • We involve and engage local people in health and care plans, from healthcare service design and improvement in the delivery of community wellbeing projects.
  • We listen to, communicate with, and involve our diverse local communities, including those who are socially excluded, vulnerable or experience the worst health.
  • We support people to look after their own health through community outreach, workshops, training, and development, making it easier to access the right information and services.

We are committed to working together across health and care system to ensure that local people and our communities are at the heart of what we do.
